Return Merchandise Form

Please complete this form if you would like to return any item purchased from HolidayLEDs.com in the last 15 days. You may also use this form to request a warranty replacement or repair for any HolidayLEDs.com products purchased which are covered by warranty.

Product returns must be in original packaging, unused, and in new condition. We will not honor returns or exchanges for opened or used items.

Refunds will not be processed until the original purchase is returned. If you are in a hurry to receive your new items we suggest that you request a refund and place a new order for the products you would like.

Once we receive your Return Merchandise Form we will send you an email authorizing the return.*Please do not send in your return until it has been approved. We will not process returns which have not been approved.

Please provide us with a day time phone number so that we may contact you regarding any questions we may have about your return.

Please provide the full name of the person who place the order.

Please select "YES" if you would like to return your complete order. If you select "YES" you do not need to itemize what you intend to return. If you select "NO" please list the items on your order which you would like to return in the additional fields provided.

Please provide the item number and quantity of each item you would like to return. For example, if you would like to return 4 sets of 41617R please write "41617R (2)".

Please provide the reason that you are returning this product.

For purchases made in the last 30 days and being returned pursuant to our 30 Day return policy please indicate whether or the item(s) to be returned are unopened and unused.

If you are returning product for warranty or because product was defective upon receipt please describe the nature of the defect.

Please let us know how you would like to handle your return. *Please note that we only issue refunds or honor exchanges on products purchased in the last 30 days which are in new condition. Exchanges will not be processed until the original items have been received by HolidayLEDs.com.
